The reason for this is that the default top line gets printed before Apple Writer starts reading text and has a chance to see any .TL commands. You must insure that the TL parameter in the print options menu is a blank line and that there isn't a .TL command in your file to supress printing of a top line on page 1. You can also use the default top line to print on page 1 and insert a .TL in the text to supress printing of the top line on subsequent pages.
